ORGANIZATION DESCRIPTION

Founded in 1912, the Los Angeles Jewish Home for the Aging (the Home) works to provide excellence in senior care reflective of Jewish values. Today, the Home serves 1,500 seniors in residential programs (Independent Living, Assisted Living, Skilled Nursing, Rehabilitation Services, and Acute Psychiatric Treatment) across three campuses. It serves an additional 2,500 seniors annually through community-based programs that include a medical clinic, hospice, palliative care, PACE (Program for All-Inclusive Care for the Elderly) and senior day care.

GRANT PURPOSE

To launch the Home’s first-ever telehealth program. The program will significantly reduce barriers to care, improve overall care, reduce costs, enable access to more specialists, and keep patients and caregivers safer during and after the pandemic has passed.

The arrival of COVID-19 on the medical landscape rapidly accelerated the adoption of telehealth at the Jewish Home, enabling us to provide care on our various campuses efficiently while reducing the risk of disease transmission. Funds provided by The Foundation will allow the Jewish Home to hire a nurse informaticist/telehealth manager to lead the programmatic and clinical aspects of bringing telemedicine to our residents. This is a critical link in the successful provision of the Jewish Home’s state-of-the-art care.
