ORGANIZATION DESCRIPTION

Founded in 1997, Partners in Care Foundation (Partners) provides streamlined access to services and programs to address the health care and social needs of low income, underserved populations with chronic medical conditions or disabilities. It provides senior services to low income and frail elders, adult day services, innovative health care and at-home care coordination for hospitals, health plans and physician groups, chronic disease self-management education programs, and develops strategic initiatives to improve health care and senior care services.

To support an outreach and engagement campaign to enroll over 370 additional low-income seniors over three years in South Los Angeles in the multipurpose senior services program, which will significantly improve care and mitigate poverty by providing essential social care at no cost to the recipients.
