For many children this program provides their sole source of physical activity outside of school, their only exposure to the arts and a much needed creative outlet. |

For the expansion of the Healthier Living: Chronic Disease Self-Management Program that supports older adults diagnosed with a chronic disease, such as diabetes or heart disease. Partners in Care will identify, prepare and certify four master trainers, who will instruct a minimum of 20 lay leaders to conduct Healthier Living workshops. The workshops are geared to help seniors achieve optimal health and mitigate health problems, such as unnecessary hospitalization and premature institutionalization.
